PTI in Chicago is excellent at executing spread orders in options... and
relatively speaking (for what they do) they're commissions aren't bad (they
consider them "discount", but are nowhere near the electronic commissions
for legging in with internet option brokers.)
Call them at 800-821-4968. Larry Kelch can set you up in either futures or
equities accounts (which include options.)
