What is "combined price and volume"? Are they not pure A-D date and can not
be used A-D line theory to analyze the market?

Nasdaq Advances: X.NASD-A
Nasdaq Declines: X.NASD-D
These are among DataLink's "Market Indicators" which I think you would have
access to (as part of indices, I guess). I don't know about puts/calls.
