In some cases the exit can be an entry;  for eg., in the case of a 
reversal signal.  But, only in some cases, because, long-term trend 
changes are not that frequent and it is not generally advisable to go 
against the long-term trend.  In such cases, price action is the 
effect not the cause for the movement.  The real cause is the 
fundamentals driving the movement.  

In short-term trading (going against or with the long-term trend), 
one can reverse you position making an exit and reverse at the same 
time, but such short-term trading involve high transaction costs and 
frequent slippage, that in real-life makes them unattractive.  

Of course, everyone is aware that the trend depends on the trader's
preferred time-frame in the trade. For me, the preferred time-frame
is the "Daily" as opposed to the "Weekly" and "Monthly".
