Thanks for all the suggestions -- quotes around the header names, 
spaces instead of commas, different date formats, etc.  But none 
of those were the problem.

Turns out there was a malformed date on one of the lines.  I 
imported it into Excel and added a column of "Date - Date[1]", 
which showed up the problem pretty quickly.

Ya just gotta love that robust TS code and great error messages.
