Do you have the custom toolbar name in the menu list? When I lose them,
the *names* are gone too, so I have nothing to click on to get them back.
The standard ones that come with TS100 are still there, and they can be
restored as you say, but any custom ones are *really* gone.
Anyone know if the custom ones are stored in files that might be saved?

I believe that all sorts of problems are much worse when a lot of
workspaces are loaded. I sometimes work with only one open when I'm doing
code work, and it seems to be more stable. On occasion, I have many
loaded (>20), and all sorts of problems seem to come up, including more
frequent crashes.
